Preventive Emergency Medical Services: A Response to the Increasing Challenges in the German Healthcare System Considering International Best Practices?

Background:
The growing challenges in emergency medical services (EMS) cannot be addressed solely by expanding transport capacities. Increasing call volumes, along with demographic changes, are driving the need for innovative care models, especially for low-priority cases that could be managed in outpatient settings. One promising approach is the internationally established Community Paramedicine model, which integrates preventive, primary care–oriented, and social-medical services into traditional EMS structures.
Methods:
This narrative review explores the potential of preventive EMS models in the German context. International best practices from Canada, the U.S., Australia, and Ireland were assessed for their applicability to the German system. Additionally, the acceptance of such models within Bavarian EMS structures was evaluated.
Results:
Results indicate that preventive approaches can reduce emergency call volumes while enhancing system efficiency and care quality.
Discussion / Conclusion:
International Best Practices and pilot projects highlight the potential of preventive emergency medical services in Germany, whose effective implementation requires regional adaptation, interdisciplinary collaboration, and appropriate legal frameworks.
